Team of the University of Oregon will be conducting an interview with
______________________________________ (interviewee).
During this time we will be collecting stories from people
on the McKenzie to share with the community through a variety of media
(including photography, video and audio). We
hope to build relationships with people along the McKenzie in order to create a
project that reflects and serves the community. By signing this release form you are
volunteering to do the following:
o
Be interviewed by a group of students and
recorded on audio and/or video.
o
Allow the photos and videos to be shown to other
groups in the greater Eugene area.
o
Allow the footage captured to be used in the
future by others; including, but not limited to, Lane County Historical Museum,
McKenzie River Drift Boat Museum and Oregon Folklife Network
As a participant in the River Stories project, you will
receive a copy of your interview and will retain rights to the material.
If you have any hesitations we are happy to discuss them and
find ways to work with you to ensure your comfort. We value your time and
contribution to the project.
